VA MedRec Tools



VA MedRec Tools are designed to provide solutions to commonly encountered scenarios for VA patients. The Ambulatory Care tool improves medication reconciliation by creating an easy-to-read master list of all of the patient's medications (including VA and non-VA medications). The Inpatient tool performs medication reconciliation at admission and discharge by creating a single master list grouping medications by indication; this aims to limit dangerous medication errors. The Inpatient + Navigator tool provides summary and note templates and integrates medication reconciliation, driving the inpatient team through the discharge process. The Inpatient + Dash Board + Turbo Note tool integrates admission, hospitalization, and discharge notes ensuring that up-to-date information is constantly available to physicians.

The VA MedRec Tools were developed by Dr. Gabriel Solomon in the Ann Arbor VA and Yehuda Bechar of Avicenna Medical Systems, and is exclusively licensed to Avicenna Medical Systems.
